Patient Qualification

X-treme Care, LLC provides services to elderly, disabled and other individuals that need in-home care and to those who needs help with personal cares and Activities of Daily Living (ADLs). We aim to make it possible for people to remain at home rather than being relocated to residential, long-term, or institutional based nursing care.
You are eligible to receive a home care service if you:

  • Have Medicaid or/and Medicare
  • Requires medically reasonable and necessary intermittent skilled service
  • Are expected to need long-term care service for at least 120 days.
  • Need to be on Medical Assistance
  • Live in a community-based residence
  • Need help with your daily activities such as bathing, grooming, dressing, or participating in the community

    • Activities of daily living refer to:

  • Bathing
  • Dressing
  • Transferring
  • Using the toilet
  • Assist with Feeding
  • Assist with Walking
  • Light housekeeping
  • Meal Preparation
  • Reminding to take medications
  • Grocery Shopping
